How to Change a Medicaid Caregiver or Care Arrangement

Changing a caregiver, agency or service-delivery option requires more than choosing a new payroll provider. Start by identifying exactly what is changing and which organization must approve the change. A transfer should protect the recipient's care and the accuracy of payment records.

Changing a caregiver is not always changing programs

Michigan Home Help clients may choose, fire or change their caregiver, but the new provider must satisfy enrollment and payment requirements. In Colorado, moving between an agency arrangement and CDASS can change employer responsibilities and budget administration. A change in service-delivery option may require a different process from replacing one worker.

Contact the responsible decision-maker first

  • Michigan: contact the recipient's MDHHS adult services worker about the caregiver/provider change and effective authorization.
  • Colorado: contact the Case Management Agency about the approved plan and proposed service-delivery change, plus the relevant agency/FMS contractor about employment and payroll.
  • New York CDPAP: use the health plan/local district and current PPL process. New York's statewide fiscal intermediary arrangement is not a choice among interchangeable FIs.

Coordinate the dates

Ask when the outgoing arrangement ends, when the incoming provider is approved and which organization pays each period. Keep the dates in writing. Do not assume an agency intake form starts paid coverage, overlap claims for the same care, or stop necessary support before the replacement arrangement is ready.

Transfer only what is needed, securely

The new organization may need the current care plan, authorization, enrollment and employment records. Use its secure process and confirm consent/authority for sharing. Retain your own service and payroll confirmations so unresolved pay from the earlier period can be addressed with the correct organization.

Recheck the financial terms

Compare the actual employee wage, benefits, payroll cycle, required duties and any budget responsibilities. A higher agency reimbursement is not automatically a higher employee wage. Do not assume every agency offers the same benefits or that a transfer completes within a few days.

Keep care continuous

Plan backup assistance and discuss any new training or safety instructions. If needs have changed, ask for a care-plan review; changing agencies does not independently authorize additional tasks or hours. Confirm the next approval rather than relying on a guaranteed timeline.
